By default, each customer can only have one phone number.  This extension allows a phone number to be associated with each customer's address.

Features:
+ Telephone per address
+ During checkout, add telephone field for new payment address and new shipping address
+ Admin can view/edit this telephone with order
+ Phone number is included in invoice
+ Customer can edit this telephone in address book
